DS-3025 is the form that was created when you had your medical exam. It should have been handed to you when you passed through your POE by the immigration officer. The embassy designated physician has the responsibility to fill the form out based on what vaccinations you had completed during your medical. If you don't have one then you will need to find a civil surgeon here in the states where they will repeat the medical and interview you concerning the vaccinations. He/she will fill out a I-693 which you will use for your AOS. Be aware without the DS-3025 this is going to be expensive. I recommend that you shop around for the best pricing. Good luck.

Thanks for that info @Greenbaum. So I hve to make sure my fiancé get it from the POE officer.

@Greenbaum yesterday, we went to local SSN office, with the form, I-94, passport and our marriage certificate.. the agent however look into her system and said.. as I-94 had June 15th, 2019 as the date and the system still shows the same date.. they're unable to issue SSN, until immigration updated their record. I assume, I need to first send I-485 form first inorder to apply for SSN.

I'M not an expert with ssn so i recommend that you go to the VJ subforum for SSN and ask your question.

Timeline

My fiance & I went for my interview to Mumbai Consulate on 16 May 2019. At the interview, my visa was approved but when I went to collect my passport, it was on AP. The consulate also sent a questionnaire, asked for resume, publications details, transcripts, etc. We sent the documents in a hurry and made a mistake (typo) in the case number and sent an old resume. We received automatic confirmation for that email. We, later on, sent a corrected email but we didn't get any confirmation automatic response for that. We have contacted the support-india team too but no help. Please suggest. Thank you

Hi, is there something special about your situation due to which they would ask for that? Resume, transcripts, etc is not on the list of documents to bring to the interview. I think it's because of my work history. I had worked on technological innovation & national security issues (defence, nuclear, etc). But all of them were theoretical studies. I didn't specify that they were purely theoretical in nature. I just got a PhD too in strategic studies. It might have come under the technological alert list (tal).

hello guys..

i have got my medical done from max Delhi, and vaccination also..

now the ds 3025 ( vaccination form ) is been filled differently by different doctors.. some mark a reason for each vaccine mentioned , some left blank ,, some do write , in summary/remarks on second page , requesting for a blanket waiver..

in my form the doctor has left blank the first 3 vaccine and also haven't written anything in the summary section.i asked the doctor and he said its complete.

 

i did a lot of research on this and found some people have received RFE after filing AOS with that kinda Ds3025..

 

i request all of you ,if you guys can tell about how your form was completed and what all was written on summary section and marked on it , so that i can go and tell the max authorities how they just fill forms so causally and incomplete.

Hi I have a few questions,

1) My fiancee (k1) and her minor child (k2) will be attending the visa interview together at Mumbai consulate.

Even-though the fiance has the full custody of the minor child would she need to have

an affidavit from the biological father of the K2 that there is no objection to relocate the child to USA?

 

2) The K2 has a different legal family name /last name than K1. Also the birth certificate

of K2 shows the old name of the K1(mother). In other words K1 has changed her name to the current name which does not appear on the birth certificate of K2.

The K1 will carry the proof of name change with her.

Is there anything else needed to show the proof that they are mother and daughter?

 

3) Does the K2 and K1 need to have the same address on their passports to show that they are living together?

If the kid is less than 18 years then it is good to have clearance from the other parent. Also divorce document should mention the custody clearly in or else it will raise lot of questions

Thanks.  Any tips about the last names of the k1 and k2 being not same?

Last name doesn’t matter as long as ur birth certificate shows ur correct name.

Yes but the birth certificate has the mother's name that is different from the mother's current name. I guess the K1 just needs to carry the document that shows her name chang. This will prove that the mother mentioned in the k2's birth certificate is the k1 applicant. Not sure it this explains my situation but nonetheless thanks for your response.

I have applied my fiancé visa for over a year, finally last week I got packet 4 letter to schedule interview, but every-time I check it shows finger print appointments but no interview appointment, since last night the dates on the calendar showing have been turning light blue im assuming people are booking those dates but for me its showing no appointment avibale, its not even showing on left bar "first available appointment" anymore, has anyone scheduled appointment for Mumbai recently, today is July 13, 2021
