I've found the I-693 forms that are to be filled out by a doctor for the Adjustment of Status medical exam, but since my husband entered on a K-1 visa, and had his medical done in November of last year as part of that procedure, he does not have to have ANOTHER medical exam. Am I correct?

On the VJ Step by step guide to filing, it says that I am to submit a I-693A, vaccination suppliment, not the entire medical report.

How do I get that? I'm confused.

Thanks for the help guys and gals :)

It's here: http://www.cdc.gov/ncidod/dq/pdf/ti-03/appdx-a_693vacc.pdf but a civil surgeon has to transfer the information to it and sign it. Most will give it to you in a sealed envelope.

You're right, a full medical is NOT required though some civil surgeons will try and tell you otherwise. Do you have your vaccination worksheet from your medical in Canada? When you do find a civil surgeon who will fill out the I-693A for you, bring the worksheet from the Canadian medical along with any other proof of your vaccinations to the surgeon.

His forms have to be mailed by the 1st. Eek...I'm freaking out now. What is the civil surgeon going to transfer the information from? All of Thomas's info was mailed to our local office. Do we have to get his results from there? If we do not have the worksheet you are talking about, where do we get it from? Is there a fee for this other medical exam if he is forced to get one?

Why do his forms need to be mailed in by the 1st? The only stipulation of the K1, as I understand it, is that he marries within 90 days of entry.

Ask him if they gave him a copy of the vaccination worksheet from his medical in Canada. If not, then he'll need proof of vaccinations from his family doctor (or whoever he got them done by) from Canada. He can call the doctor back in Canada to see if they can send him a copy of the vaccination worksheet or call his family doctor to send him proof of vaccinations.

He does NOT need another medical. Only proof that his vaccinations are up to date in the form of either the vaccination worksheet OR a completed I-693A vaccination supplement. If he submits the DS vaccination worksheet, he might get an RFE requesting the I-693A. The fee to get an I-693A completed varies from doctor to doctor. I paid $45. Some pay as little as $10.

We were told that if his Adjustment of Status application was not recieved by the date of expiration on his visa (June 11th) then he would have to go back. Am I wrong? lol.

misa, youve always been awesome help.

Who told you that? There are quite a few people here that file after the 90 days, but they marry within those 90 days. He will technically be "out of status", but just make sure he doesn't leave the country! :)

You should however file as soon as possible to get it over with though. You have time to get that I-693A done. Have him call the doctor in Canada first thing tomorrow as well as his family doctor for his previous vaccination records if needed. And in the meantime, make a list of all the civil surgeons in your area (there's a link here somewhere for the civil surgeon locator) and call them all. Find out which ones will do JUST the I-693A vaccination supplement and for how much.

Good luck!

Thanks! It's nice to know that he will not get deported if we don't have the packet in on time. That's what we were worried about. It seems like no matter how much research I've done, I always find something at the last minute that brings about a delay.

We just had it done last week, all you need to take to the civil seurgeon is the passport with visa, and vaccination record, the doctor has the form and will do it for a fee.

Bring the instructions for form I-485 and you can show the doctor where a K-1 does not need another physical exam done, just the vaccination supplement.

I took the instructions, and the doctor stopped insisting on a physical when I pointed this out.

OUR TIME LINE Please do a timeline it helps us all, thanks.

Is now a US Citizen immigration completed Jan 12, 2012.

At the K-1 medical exam, you should have been given the "Vaccination Worksheet". If not, call the facility where the medical exam was done and request a copy be either mailed or faxed to you.

Then, take the worksheet to a civil surgeon and have them transfer the information onto the "Vaccination Supplement" which is required for the AOS application. We were charged $35.00 for this service.

You do not need another medical exam as long as it has not been more than a year since the K-1 medical exam.

Good luck.

We called USCIS the other day, and we were told that we did not have to send it off. They would send us a request for evidence if they needed it. That's weird, but then again, that's the way this whole experience has been.

This is quite true, however I would still just wait until you have everything before sending the packet personally. Still, if you're worried about going out of status, this would be one way to avoid that.
