Hi everyone! I'm a K-1 Visa applicant (beneficiary) and I went to get my physical exam on friday. At the doctor's office they gave me a sealed packet and a separate sheet: the DS-3025 (Vaccination Doc Worksheet). When I was reading it I noticed that on the "Vaccination Documentation" section there are two options: (1) "Immigrant Visa or Parolee applicant completed vaccination requirements" and (2) "K Visa applicant voluntarily completed vaccination requirements".  The doctor checked #1, and I think he should have checked #2, since Im a K-1 applicant and the K-1 is a Nonimmigrant Visa. Im worried this is going to give me issues at the interview or at the port of entry. What do you guys think? Did your doctor also checked the #1 option? Thank you.

The same thing happened for my now hubby's form, box 1 was checked. There were no issues at the interview or port of entry. We sent the AOS packet in April, and I'm hoping that everything is OK. At least all the shots are done, so hopefully that little box won't make much of a difference.

Perfect! Thank you very much for your answer. Good luck with everything.

Hello everyone!

I would just like to know if anyone knows where in Costa Rica could I get all records of vaccinations I've ever received? or if possible, a test of sorts that is not very expensive to determine this?

I went to la Caja to ask but with no success, lady said they: "Don't do that"

I have a small booklet which kept shots I had gotten all the way until I was 11 or 12 I believe, but I really really don't want to pay for any more vaccines as I've noticed that they're quite expensive here.

All we had was the book too from childhood.  They accepted that without an issue at the physical.  In order to avoid costs, my husband went to his local hospital and got the remaining vaccines he needed for free before the physical exam date.  I don't think there is a way to retrieve old records unless your previous doctor or hospital had them electronically.  Where my husband lived at least, that did not exist.  Best of luck!

Thank you so much for answer.

Would you be as kind as to ask him how we went about going to the hospital to get the remaining vaccines? Did he needed an appointment? If so, how did he make it? And in which area did he got the vaccines? General medicine or a specialized ward?

Thank you once again for the answer , I feel this process almost ending and it's a little unnerving how much goes into this whole thing.

He lived in Quepos and went to what he said was just Seguro Social, like a small clinic that is associated with the hospital and they do vaccinations and small things like that.  No appointment, but you’d have to find out where one is located near you! Yes this process is long and tough, but it’ll be worth it!

Hello everyone, I know this thread is pretty much dead but I'm panicking.

Can the I-134 be a scanned copy or does the original have to be brought in ?

We're almost at the end of this whole thing and it has been nothing but stressful @__@

It says it requires a "wet" signature in black so that would be mean original of the signature page.

Well I went to actual government website and it does not say it specifically...there is a forum on visa journey and it seems that the debate on this is mixed....  Forum is: Form I-134 must have an original signature?!?

The P3 that the embassy sends you doesn't specify that it needs to be a wet signature.

Hello hello hello!

I just got recently approved today at my interview and I couldn't be any happier 😊!

I was just wondering if you guys could give me some insights into when you received your visas as me and my fiance have some plans and we would like to know around when should we expect to receive them, they told me in the interview that they'd give me the visa around 10 business days but have you guys got it any sooner?

Thank you in advance!
