In the question about my savings and checking information, do I need to include information about my Roth IRA, IRA, and 401(k) accounts?

 

And in the area about real estate value and mortgage or other debts, my ex and I have a house and mortgage, however, while my name is still on them, she takes care of them. During the divorce the house is officially hers, but we just haven't gone through the paperwork to remove my name from them. Do I need to include this information even though it is hers to handle?
