We applied for I-485 in April 2017 and we just received an Interview notice for AOS in Sep 2018. Does the medical examination and vaccination supplement need to be redone since it has been more than a year since the original one was made?

Yes because it expired the year after.

 

I didn't do my medical when we submitted the 485 for this reason.

There is no information that says that the overseas medical expires, it only says it needs to be submitted within one year of the date of the medical. After that it doesn't expire.

 

So technically, no you don't need to redo the medical.

 

However, some USCIS employees don't know their own rules and still requests a new medical even if they shouldn't. They seem to mix up the medical that is done within the US (this one is only valid for 1 year) with the overseas medical.

 

If I were you I would save the money for now since you don't know for sure if they will ask you for another medical or not. A new medical is expensive and if it turns out it's not needed, then that's a lot of money for nothing.

 

 

"4. K or V Nonimmigrants Applying for Adjustment [26]

 

K and V nonimmigrants applying for adjustment of status are not required to repeat the medical examination if the application was filed within one year of the date of the original medical examination, and:

 

The medical examination did not reveal a Class A medical condition; or 

The applicant received a conditional waiver in conjunction with the K or V nonimmigrant visa or the change of status to V and the applicant submits evidence of compliance with the waiver terms and conditions. [27] "
