I hope when you said help you really didn't mean do this for you. It would be impossible because we do not have access to the same materials/documents to answer. I recommend that you use this letter as a checklist and begin answering the requests. If you have something you don't understand then ask that question. Compile a cover letter identifying what your are returning and place the evidence behind the letter in the same order as they are asked for. DON'T add more then what is asked for or that can problematic for your case. If you send everything they are asking for and that material answers their questions then you will more then likely go on to getting your approval.
