First, they prefer that your case is interviewed within that four-month "window" indicated on the NOA2. If it is required, the NOA2 can be extended up to a year by your Embassy but with good reason and the Embassy doesn't need to extend the NOA2 for the "one" year from expiration. The new extension, if approved, will be for an additional 4 months. Extensions are in the form of 4-month extension with each approval till you reach one year. If you need to go beyond a year, then you must go to the end of the line and begin the process from I-129f over again and pay the fees. You must provide a reason for the NOA2 extension when requesting the extension from your Embassy.

Timeline

I got the email from NVC that was case was being sent to the Embassy on Oct 3rd but it said "in transit" the day prior. I'm hoping it will reach the embassy soon, I don't know if we should get the DS160 going and schedule the medical exam or wait for it to arrive to the embassy first.

Yes, you can travel all 50 states (include Alaska and Hawaii). Carry your passport and your marriage certificate. Passport is needed to board an airline flight since you do not have a shiny new "Real ID". https://www.tsa.gov/travel/security-screening/identification

Well, the first thing is you need to have a copy of the P3 to ascertain whether your Embassy will allow you to take those steps you are inquiring about. And since most here are trying to get their arms around their requirements, I highly doubt they will be able to answer your question. Not all Embassies operate the same. So, I recommend that you begin using the sub-forum for your country/region and ask questions their where there are others who are familiar with SK.

I am talking about the fourth question under travel information base on my guide that's where the nvc # should go I could be wrong but know for a fact it won't allow you to leave it blank

They use so many interchangeable names for numbers required that it can be confusing. Try the case number because it is asking for the NVC #. WAC# could be the other possibility. 

 

Here is a good primer for completing the DS-160. http://www.visajourney.com/content/ds-160-help-and-frequently-asked-questions-faqs Also, I have these two from two Embassies. The DS-160 is the same for most Embassy's. When you are filling it out for your country only country-specific questions may appear. So if the example doesn't show it, that goes for all links I share here with you, then don't panic as it isn't a requirement for your country.
