When can one ask for an early interview date?

I am in process of submitting AOS and DS-260 to NVC. I have seen some posts here on vj where folks have asked for an early visa interview date and were successful. My questions are:

 

1. When can one request such early interview? After Case complete or once the visa interview date has been assigned?

2. Is medical required for requesting an early visa interview date? If so, can one get the medical done before visa interview date is assigned?

Timeline

1.As far as i know, unless you have a valid reason you can contact the Embassy after your interview date has been assigned by NVC.

2. No. You only take the medical after the interview date has been assigned.

Does one need to contact the NVC or the embassy for requesting an earlier interview date?
