On an approved application your NOA2 is good for one year with extensions throughout that year at 4 months at a time. I'd suggest calling USEM when your "expiration" of the NOA2 is near to have it extended. If all the stars line up, you can do the Medical on Monday and Tuesday, CFO on Wednesday and the interview on Friday. Then return home to wait for the visa to be delivered. Once delivered then return to any CFO location and have the certificate placed in the passport. Then off to the airport (you can even get the CFO certificate installed at NAIA) to fly out.

Your fiancee has up to a year to complete the visa application.     The embassy will automatically extend the validity of the petition during this time.

Process is; pay visa fee, schedule interview, complete medical, complete interview .. then CFO

With the new baby ( Congrats!!) your fiancee will need to complete CRBA  (and I highly suggest getting the Philippine passport also)

No need to call the embassy regarding the extension.

(Don't forget their new baby... CRBA)
