Do you mean DS-260? Because DS-160 is for non-immigrant.

 

DS-260 and I-864 are not expired after being accepted.

 

However, if you are going to move to a new US address after entering the US, or you need to correct some wrong info, you should notify the Consular Officer in the interview. They will help to edit DS-260 for you.

 

Only police certificate is expired after 12 months and needs to be updated at the interview.

The police certificate and medical reports expire 6 months from the date they issue if you don't get your immigrants visa within 6 months then Ghana US consular   will gives you a new forms for medical and police certificate again so you will do them again after 6 months.
