I am not sure if you posted this before or after private message me. Like I said, this isnt a form, she needs to get the original birth certificate notarized. She needs to walk in to the Cartório and request it if she doesnt have the original one.

The beneficiary needs to present their original birth certificate (certidão de nascimento) with a simple copy at their interview.

Newer birth certificates look like the one you posted, but it is not a form you file. It’s a document from a cartório that registers their birth, everyone should have one.
