Medical Questionnaire... Question

Hi all :)

I'm from/live in the UK if that makes any difference to the below :)

Doing some research and I have the questionnaire that you fill out before going to the medical exam. We haven't officially started the process yet though I like to be prepared. 
I noticed two questions I am worried about. 
Have you ever had any mental disorder or depression? And Have you ever caused deliberate injury to yourself or others?
Obviously I want to answer honestly and my medical records will show that yes to the first and to myself to the second and wondered if this has effected anyone else's result or not? I don't think it should however I just wanted to put my mind at rest really either way.

They already know your history so no point but to answer honestly (and the penalty for doing otherwise is a permanent bar).

Answering yes does not mean they will refuse the visa. They may (or may not) require a psychological evaluation to ensure you are not currently a risk to yourself or others.

1/2/18: EAD + AP approved (no website update), 1/5/18: EAD + AP mailed, 1/8/18: EAD + AP approval notice hardcopies received, 1/10/18: EAD + AP received

9/5/18: Interview scheduled notice, 10/17/18: Interview

10/24/18: Green card produced notice, 10/25/18: Formal approval, 10/31/18: Green card received

5/1/2017: Case received by Manila (1 week embassy transfer??? Lucky~)

7/13/2017: Interview: APPROVED!!!

7/19/2017: Visa in hand

8/15/2017: POE

This has been discussed often in the U.K. forum. The short answer is you can get your visa like all the rest did. You'll need to take some extra documentation to your medical exam.
