Exactly. To assume every man in Pakistan wants children is so ridiculous, especially in this day and age. Pakistan is 500 years behind the world. They think just because the person is coming from Pakistan, they can apply customs that are out of touch with this day and age, even accounting for past history. Not everyone lives like that now.

This is common. The CO doesn't have to look at any extra evidence given. They can generally make a decision off the initial I-129f petition they receive from NVC. So it must have been relatively weak

Spend more time together, gather more evidences of your relationship and apply again. Make sure you know each other good enough so you can answer basic questions about your relationship.

It's obvious when a person gets into the interview that the decision has already been made.

I have a feeling that most are straightforward and it is just a tick box exercise, there will no doubt be some that could go either way.

 

They are also going to be aware of local norms which can be very different to the US.
