Vaccination documentation worksheet

Hello dears , we will be on our way to the usa by tomorrow (immigration visa ) for me and my daughter, we went to her doctor today to get her vaccinations record and he write down on her vaccinations documentation worksheet (copy) and stamped by the hospital we made the medical exam in , I don’t know what to do and if there will be a problem regarding this . We are traveling form egypt btw . TIA

I’m not sure I understand what you are asking. You had a medical for the immigration visa and the panel doctor approved by the USA gave you any required vaccinations and a vaccination worksheet, correct?

 

I’m not clear about what happened at the hospital that they stamped the worksheet, is that a different doctor, a different vaccination record, what? The only one you need for any purposes in the US is the one you get at the medical, that’s why they give you a copy of it.

Our private doctor wrote on the copy of the vaccination worksheet . Am afraid it will be a problem at the airport or they might don’t need the copy ? Only the original one at the visa packet?

It won’t be a problem for entry, they only need the one in the visa packet. The copy you got is for your own use. If your daughter will be entering school, you’ll use your copy to show the school what vaccinations she’s had.
