Which type of Visa? Once they stamp your visa page you have to enter usa within 6 or 3 months from the date of your medical examination. You need to check. Most cases 6 months.

Regardless its 6 months from medical. 

K1 need to have their interview booked before the noa2 expires if possible, which is 4 months from noa2.  But they use the Ds-160 not the DS-260.

 

IR1/CR1 and other immigrant visas have 1 year from IV payment date to complete the Ds-260.
