Where to find doctor for I-693?

Where can I find doctor's in my area that are approved to handle I-693 for my wife and step-daughter? Also, they entered US on K3/4 visas and had all medical exams before entering the US. Unofortunately we can't find the documents we were provided. Is there a way to get a copy of the original documents or will we just have to go through the process again with I-693? Any comments would be appreciated. Thanks.

We're checking this out too. I know that K-1's/K-2's don't have to do the medical exam again for AOS, they just need a vaccination supplement. My husband said they kept everything except the X-rays at the consulate and/or point of entry. I've been told by other VJ members that this requirement also applies to K-3's/K-4's because K-3?k-4 is also a nonimmigrant visa. I'll be calling civil surgeons in my area this week to check this out. I'm going to ask them if we can update his vaccinations with our family doctor since it's covered by our health insurance, then have his vaccinations approved by the USCIS approved Civil Surgeon. I'll keep you posted.

The only thing that you usually have after your POE is your DS form filled in by the foreign CS and your chest xray. If you do not have the vaccination form "medical worksheet 2"(DS). then you should be able to contact the doctor that did the foreign medical exam and request a copy. Sometimes this form works when people are adjusting status. Others will get an RFE and be asked for the supplemental form to I-693. Some CS will use the DS form others prefer to use your own vaccination history and not "trust what another doctor says" .

You most certainly can get your vaccinations from wherever you choose. Usually that is a cheaper option. From what I've read too that the vaccinations that are in a series like Hep B... you only have to show that you've started this, not necessarily that you've finished it.

May 31, 2012... Express Mailed N-400 application to the Nebraska Service Center for military spouses filing overseas

Jun 11, 2012... NOA date and Cheque cashed Day 11

Jun 28, 2012... Placed in line for interview scheduling Day 28

Jul 20, 2012... Email from USCIS London for info to schedule interview Day 50

Sep 1, 2012... Moved back to the USA

Sep 12, 2012... Email from USCIS London scheduled for interview/oath Sept 28th in the UK(will be day 120)

Nov 26, 2012... Had submitted change of addresses. Notification that I have again been scheduled for interview Day 179

Dec 1, 2012... Letter arrived Day 184 with new interview date Jan 4, 2013 (Will be day 218)
