Me and my ex husband were married for 3 years but recently got divorce. I have a lot of evidence of real marriage but I am really confuse about how to complete the forms. Mostly. I need to know in section 4 where it asks for spouse or former spouse information. Should it be blank or should i submit his information? The same about section 7 and 8. Should it be his information, mine or blank?

Part 8 signed by spouse (not in your case) since you file with a waiver
