how to make affidavit stating we are living with her parents in their house  since we  got married  can anyone send me or upload format or template of it for sending along with i 751 application?should those letters need to be notarized?do i need to have separate letters from both of them or having just one letter from father in law would be sufficient as he is head of the house?

Yes the affidavits need to be notarized. I used a few for my RFE.  I used the example form from here http://www.visajourney.com/content/examples/
