The answer for first is it yes or no my spouse is overseas and  I am here USA .I am filling DS 260 and I find that question.

2) about vaccination I do not is it yes or no

1)Do you have documentation to establish that you have received vaccinations in accordance with US law ?yes/no

2)is your spouse immigrating to US with you?yes/no

I'm sorry, but I don't understand what the problem is. These are simple questions, and only you will know the answers.

1) Means of the beneficiary will be migrating with her spouse. Since your spouse is the beneficiary,the answer should be NO.

 

2) Answer NO with this explanation. "All Vaccinations will be taken during medicals before my visa interview."
