Long story short.  My mother in law came to america three months ago.  We were expecting her first initial permanent resident card.  USPS tracking # indicates that the card was delivered.  We haven't received it.  It's been almost a month since it was delivered.  At this point, we have lost hope that it will be returned in the mail.  It's possible it was mis-delivered/stolen.  

 

Has anyone gotten a waiver for a replacement permanent resident card?  Has this happened to anyone before?  What should we do?

This had happened to us during AOS. Unfortunately, there is no waiver.

 

First, we went to our postal office Postmaster and asked to track who actually deliver it. Back then, the postal carrier attested that she delivered the mail to our mailbox. So then, we called USCIS and had to wait for 30 days to see if the green card will be delivered back to them. After 30 days from the date of the tracking number, we then filed I-90 with the filing fees. Unfortunately, your fee for I-90 is now $540 including a new Biometric.

 

For this lesson, we rent a postal box at our local UPS Store and have all our immigration papers to send to this mailing address. At least, when USCIS send the replacement green card, 10 years green card, citizenship certificate and passport, there will be someone to sign it.

I've made the trip to the local post office.  They went as far as showing me the gps coordinates that say it was delivered to my house.  I called USCIS customer service and they told me to wait 60 days to call them back to see if the permanent resident card turns up.  I've never had mail/package missing before (that I know of).  What luck huh?  

 

How long did it take until you received the replacement green card?  Was it the first green card or renewal?

It was the conditional green card that was lost or stolen. At the time, it took about 8 months to get replacement.
