RFE for I693) How can I have my Vaccination record?

Hello, I got an RFE and need a new Medical. I have set an appointment to USCIS Civil surgeon, but they said I need to bring my immunzation records.
Which I dont have. I think I didnt make a copy of it when I got them from Philippines Clinic. Now I dont know how am I gonna get a copy of this?
Does anyone here dealt with this kind of stuff before?

Weren't you given a DS-3025 after your medical overseas?

That's the only "foreign" document civil surgeons will accept to transcribe your vaccinations in part 9 of form I-693.

If you had one and sent it in your AOS Packet without making a copy, you should try to get in touch with the medical facility in the Philippines where you did the medical examination and explain why you need a copy. Hopefully they will help you. 

 

Other options you have (I recommend you speak with your CS about them instead of deciding on your own)

1) get the vaccinations again, if possible (worst case scenario)

2) get immunoglobulin blood tests to measure your level of immunity against all the diseases for which you are required to have a vaccination according to USCIS and CDC

 

Why are you required to have a full medical again, if you don't mind me asking? Was it more than a year ago?

Hi, thank you for replying. I thought I have made a copy of that DS 3025 but I actually didn't. Yeah, It was over a year already. So I need to redo it. I'm wondering how much would it cost normally for the vaccination to be done again tho.

I came to the States on my K-1 recently and I have just submitted the AOS. Before doing so I had to get a second MMR. My health care plan covers it entirely, but the waiting time to schedule an appointment with it was going to be several weeks, so I had to cover it myself because I wanted to submit the packet ASAP. I did it at Wallgreens for $99, and then saw a civil surgeon who transcribed it on Form I-693. 

If you have a health plan you could try to look in advance to figure out if it would cover new vaccinations for you. In most cases they are covered. In that case try to check if it would cover the immunoglobulin tests to verify your immunity too... if you self-pay they can get super expensive. 

Cotsco is also an option for vaccines... prices are great, way less than other chain pharmacies. If you have a membership get in touch with your nearest. In my case they didn't have the MMR available but I would have saved half of the money. 

It will also depend on your Civil Surgeon... some accept to transcribe vaccinations done elsewhere, some will not.

I had to call many to verify that. Make sure you know exactly who you are dealing with before taking any action. 

I still recommend you get in touch with the panel physician who did your medical in the Philippines to check if they can release a copy of your DS-3025.
