NBI Problems

US Embassy in Tokyo requested my Filipina fiance provide an NBI with both her married name and her former married name. She was married to Japanese man and they had spent time in Philippines after marrying.  She went to the Philippine Embassy in Roppongi (Tokyo) yesterday.  She was told that she could only apply using the name on her current passport, which is her maiden name.  I saw a sample NBI Application online and there was a block to put "Other Names/AKA"  I told her to put her married name in that block.  She called me to say that there was no such block on the application they gave her to fill out.  This is the only piece of documentation we need to satisfy the US Embassy's request for RFE.  I am at a loss as to what to do. We have come so far in the last 18 months to comply with all their request for documentation. She had her interview in Jan 2018 and was denied due to missing documents. Does she have to go to the Philippines herself in order to get this done?? Can anyone please help with any information on how to proceed. Thank You.

I can't believe the Philippine embassy isn't aware how to request an NBI with the AKA.

Unfortunately I was not there, when we applied in May I was there in Japan and I tried convey to the consular agent she needed NBI with both names. All he told me is she can only apply with name on her passport. When I tried to explain again,  he told me to sit down or he would have security remove me from the embassy.  She went and called me and I asked her if there was a box to put other name/aka. She said no, there was no place to put that in. I will talk to her today and try to get her to go online or I will do it for her and then set a date in the future, giving her enough time to get time off work and fly to the Philippines to obtain it.  Such a  mess, and I have called the embassy before and tried to ask a question but they were clueless, told me to contact main office in Philippines and they could not answer my questions. Incompetent is being nice.

Ya it can be very annoying trying to get this stuff done long distance.    Hopefully  it can get handled online.   Suck to have to fly the 5 hours to the Philippines and then back again.

My first thought is that she tried to create two separate NBI searches.  This should just be her current name and there is a space to write AKA's.  I think I've seen single NBI certs with 5 or 6 names on them.  Rose's had 4 names on hers.  It all has to be filed under the current name though.

PHILIPPINES ONLY!!!  CFO (Commission on Filipinos Overseas) INFO - Can't leave home without it!

 

PDOS (Pre-Departure Registration and Orientation Seminar) is for ages 20-59.  Peer Counseling is for 13-19 years of age.

It is required to have the visa in their passport for PDOS and Peer Counseling.

 

GCP (Guidance and Counseling Program) is for K-1 Fiancee and IR/CR-1 spouse ONLY.

I will say she is a very simple woman, she is timid and once the consular agent told her she could only apply with her current passport, she took that as meaning she could not add additional known names. Guess I will find out in a few hours when I call her. I have seen sample forms and every one I saw, had a spot for "Other Names/aka  or Aliases.
