Hello guys,

 

Just a question in regards to part 4 on the form when it asks for beneficiary's travel history into the US. Now, the question asks if the person is STILL in the US to fill out the following

what if the member visited the US and now returned back to their home country.

 

Do I have to fill out items 46 a. to d. even if my parents are already back home?

So, I guess the passport number we put on the form should match the number of the actual passport not the one that has the US visa on it, correct?
