Does this mean from when it gets to the beneficiary's contry USCIS? - Does the beneficiary have to do medicals, apply for visa and do interview within the 4 months?

Sorry if a silly q, just want to be sure I understand this correctly.

Check the approval letter from USCIS and it should have the expiration date on it.Yes the beneficiary has to do all within four months.

Yes, but the date can be extended by the embassy up to 12 months after the NOA2 (I-129f approval) date. Some embassies do it automatically, some do it by request, and/or some require you to provide an updated letter of intent to marry.
