Ok... I finally went to Washington County Sherriff's office on Friday, they gave me a Certified copy of my "Police Report" which by the way for a Sherriff is called a "Good Conduct Report".  I am 100% sure this is what I needed on my initial form.  I listed my one and only misdameanor offense with Court Judgement, but they need to see that exact arresting body with a clean report... So in this case Washington County Sherriffs report.  I originally sent only the "Portland and Hillsborro" Police Reports as all clean but because the arresting agency for this stupid offense back in 2001 I had to get that agencies report..... I hope this helps someone else.
