The medical Exam should already be with USCIS, when you entered in the United States and handed them envelope, that envelope should have your Medical Exam. Now, when its time for your AOS interview, they might ask  you for a medical exam if the old one is more than 12 months old. This is what happened to us, we went for the interview and the Officer asked my wife to submit a new exam in sealed envelops from USCIS approved Civil surgeon.

You could err on the side of caution and just throw a copy in the packet. The number of threads on here you see of USCIS losing paperwork, or whole petitions, I don't think it could hurt. I know I am including a copy of my DS-3025 when I get to AOS sometime in the future. I don't ever remember someone getting penalized for sending something extra. Even with my I129 I submitted copies of both our passports and copies of both of our birth certificates even though for me one or the other is sufficient to prove citizenship, and so far no RFE for sending too much information. will know here in another month or so, well hopefully, LOL.

Here on a K1? Need married and a Certificate in hand within a few hours? I'm here to help. Come to Vegas and I'll marry you Vegas style!!   Visa Journey members are always FREE for my services. I know the costs involved in this whole game of immigration, and if I can save you some money I will!

Okay thank you for your replies, so just to be clear we dont have to send a Copy of DS-3025, Vaccination Documentation Worksheet from St. Lukes (medical examination in Manila) with AOS , but can send if we want just to be safe?

Thank you

The medical Exam at St. Luke, if not send directly to the US Embassy, it will be handed to you in Sealed envelope to give at the K1 interview. You need to keep copy of the DS-3025 you might need it in the US after your arrival and if Medical Exam is required again it will be a prove that you had all vaccinations completed.
