DS-3025 or I-693? (K-1 AOS)

Hi all - I believe this question has been asked many times but I still want to ask one more time to double check and make sure. I got my medical examination along with vaccination prior to my K-1 interview in April. The doctor issued me a copy of DS-3025 with the box "K Visa applicant voluntarily completed vaccination requirements" ticked. I still need to get one Tdap shot but it looks like that I got a B waiver (insufficient time interval). A couple of vaccine boxes are left in blank (Polio, Rotavirus, Hib, etc.). Now I'm working on my AOS and was wondering if I-693 is still needed. Based on my research, it seems that I don't need to submit I-693 but can file a copy of DS-3025 (and even this is not NECESSARY since the original was already included in the pack at POE). Could you please kindly confirm? Many thanks!

September 13, 2011: Paid IV bill (payment portal showed PAID on September 14, 2011)

October 3, 2011: Emailed checklist response (checklist generated due to typo on Form DS-230)

November 10, 2011: Interview - APPROVED!!!

January 28, 2014: Production of new Green Card ordered

December 18, 2023:  Filed I-90 to renew Green Card

December 21, 2023:  Production of new Green Card ordered - will be seeing USCIS again every 10 years for renewal

I am not going to give any misinformation here since there was a debate about Medical Exams and its validity (12 months) but I will give you my experience only. 

My wife had her Medical Exam in Singapore around October of 2016. We filed our AOS i-485 on May 31st or 2017. 

June 9th of 2018 we went for the Green Card interview. The office asked my wife for her Medical Exam. We told him we already submitted one. He told us that Exam already expired and we need a new one. By the following week, we had one done and it was submitted. Now, if all your vaccination are in order and complete no need to worry about them, but if they are missing then they have to be completed and signed by CIVIL SURGEON. 

 

Now, some people said that no need for another Medical Exam if a K1 already submitted before. In our case, that did not work, why, some said the officer made a mistake and he was wrong. We just followed his instruction and today the Green Card already mailed to us, four days after we submitted the New Medical Exam. 

Would they ask you for a new one by the time you are going for an interview? I have no idea...I know someone posted something in that regards from USCIS page.

...so, did you have to pay twice for the I-693?

Yes, we had to pay $199.00 for the Medical Exam in our area. The vaccination was not needed because it was complete back in Singapore, but we had to give the CIVIL SURGEON a copy of the Vaccination copy (DS-3025) record so it would be submitted inside the envelope signed and approved by the Doctor.
