Did any H4 visa holder win the lottery?

AOS speed doesn’t depend on what status you are adjusting from. The speed of the process depends both on your case number and on your field office. Typically AOS for DV will take a good few months after you are able to send in your package, but it can vary widely. In particular FOs may feel less urgency at the beginning of the FY than near the end, where they will usually move faster than usual to get it done before the deadline. Some FOs seem to batch their DV cases for processing so may also depend whether you are first or last to be in a particular batch. Etc. are you DV18 or 19?

 

What do you mean, what should your spouse do? Does s/he want to get a green card too? If so obviously they need to file AOS as your derivative. If not they can just stay on H1B.

Again...assuming she wants to adjust status, yes. I confess to being a little confused at your questions.  Are you unsure if she needs to submit her own from (in which case may I suggest you read the instructions for the form, which clearly states she must) or does she want to do something else (not adjust, adjust through employer, whatever)?

I played the 2018 lottery and a visa will only be available in August and as you know I only have until end Sept to get things done.

Yes it must be with you. You (principal + derivatives) would all interview at the same time. That’s how all US immigrant visa interviews work.
