On the first page for the i751 it has 3 boxes for her name to be put, there is always a problem with vietnamese name because they go from Last-Middle-first instead of first middle and last. When she arrived in the usa her name is 

First- P

Middle- Q

Last- N

 

Her Actual name is 

First- Q

Middle- P

Last- N

 

do i just go ahead and use what the usa consulate used as her name and whats in her passport? P Q N i was told to not change anything? this is how her name is for major documents also.

On Visa, it doesn't separate first and middle. It is just last and given name. In your case,

Last: N

Given name: Q  P (using her Vietnamese first and middle name)

Her green card also follows this same format. 

 

If her visa and gc given name is 'P  Q', the consulate really messed up her name. You have no choice but using this messed-up name in I-751

every vietnamese name is like this believe me it's a problem all the vietnamese filiers have been trying to figure out

it is truly unfortunate that the consulate mixed up your wife name like that.

 

In her IV application, have you entered her given name by correct order (i.e., First Middle)? I  have seen some vietnameses entering their IV application 'give name' in wrong order (i.e., Middle First), so their Visa and GC having swapped First and Middle.

they go by what the passport says it's a ongoing issue with vietnamese and name orders
