I've called several civil surgeons today (Aug 27th 2007) from the USCIS approved list and thought I would post my results in case they might help someone else. I was calling JUST for the I-693A vaccination supplement. These prices, from what I can gather, aren't set in stone and really seem to just be made up on the spot so undoubtedly your mileage WILL vary.

If you want to pay only $10, use the CS that I used. he is in Austin Texas but you just MAIL everything to him such as your DS-3025 ... he will then transfer everything for you to the I-693A vaccination supplement form. This is much easier and convenient if you guys are having to travel out of your way for this.

Wow, I found a local guy who would do the TD vaccine AND the I-693a form for $25. Not bad. However ...

Some people have reported success with ONLY the DS-3025 (that's right, NO I-693a form sent), and we were pregnant in first trimester so decided against getting the missing TD vaccine and just sent the DS-3025. I'll let you know if we get an RFE. With the deludge of I-485 applications just before the fee increase, by the time they get to ours the baby will be here already!
