Please help! Question regarding i865

We have to mail in our I865 form tomorrow and I have a question!

 

Do I have to put N/A in all the blank spaces? Such as middle name( we both don't have middle name), all the other blank spaces for interpreter, extra sponsored immigrants section?????

 

AND I can just see s this form(5pages) by it self right?No cover letter and Such?

I have No idea how to edit this, I meant to say 'send not see s. Sorry! 

(Btw i865 I'm filing is for address change)

You can leave it blank or type N/A it doesn't matter. A cover letter is not required until or less you are mailing the form late.
