Hello, I just wanna ask about Renewal of Passport in Philippine Embassy Outreach Program.

Last month I emailed my requirements to get an appointment to their Outreach Program

but unfortunately I didn't receive any confirmation for appointment and I'm not included on their final list.

I want to renew my passport (almost expired) and change my last name at the same time to my married last name.

My current passport last name when I was still single.

Documents I sent:

1. Renewal Passport Form ( I used my married last name here)

---After I failed to get an appointment I have doubts if I filled up the form correctly...

Question 1: Do I need to fill up the form base on my passport last name or my married last name?

2. Copy of Passport Biographic Page

3. Birth Certificate

4. Report of Marriage

5. Notice of Action (letter of extension for my green card), I didn't include my green card copy since it is already expired but ROC already filed.

Question 2: Do I need to send/mail the passport fee?

Hope you can help me. Thank you so much!

1)  complete the form using your current name.

2)  No not for getting an appt for the outreach event.  You pay it at the outreach event.

I don't think the reason you didn't make the list was because of the forms/documents, you weren't quick enough getting on the list

(I know some just show up  at these events .. hoping to get taken.   Not sure how far the outreach event is from where you are, but that may be an "option" )

What Outreach were you trying to go to here in the USA? I know for a fact that the Outreach programs fill up really quickly with appointments for those days they are there. I was emailing them every single day just to get one appointment and I got one the last day in the afternoon that they were in NC.

You can try to go there to see if they will see you but I seriously doubt it in all honesty.

1. If you are doing passport renewal like your married then everything needs to be in the married name.

You need to do the ROM either at the same time as the passport renewal or beforehand, and that can be done in person or by mail to the Consulate that you got married at.

Bring copies of your expired greencard AND copies of the NOA extension letter showing that you do in fact have status in the USA.

2. You can do the Report Of Marriage(ROM) by mail or in person at the Consulate that is in the area of where you got married at. The passport renewal NEEDS to be done in person at either the Consulate or the Embassy in Washington DC, this requirement can not be waived at all.

I sent my requirements an hour after they posted the schedule in Nashville TN Outreach.

And on the renewal form I used my new last name not coincided to my passport.

I forgot to ask this to my first message...

Should I send a copy of my expired green card?

Yes, send a copy of your expired passport when doing passport renewal.

Edit. You do not send this stuff to the actual Outreach, you bring it with you when you go there. The ONLY thing that you can do by mail is the ROM to the Philippine Embassy in Washington DC if you got married in TN. Passport renewals have to be IN PERSON that is the new requirement set by the Embassy.

I had to sent in the email literally EVERY SINGLE DAY until they opened the Raleigh Outreach up and we only got a appointment on the second day in the afternoon. So you can try to go in person to see if they will see you but other than that you will have to go to the embassy in Washington DC.

Not necessary to send the expired green card IF you sent a copy of the I-797 NOA from filing ROC .   No harm in sending a copy either, but the consulate will be more interested in the I-797

I always bring more just in case they might ask for it.

I agree .. .thus my "no harm either..."  ;)

Item 4. in the requirements to prove not a USC =

                      4.  Proof that the applicant is not a US/Foreign citizen (Original and two [2] photocopies of any of the following: greencard, visa adjustment, notice of action, work permit)
