When I send off my packet to the USCIS I send form I 184 affidavit of support off and a bank statement. My question is do I have to fill out that form again or just take copies of the ordinary form? Please if someone can help me with this question I will really appreciate it.

You don't supply any affidavit of support with the initial I-129f petition packet. This is supplied after the US side of things to the consulate when the interview takes place. Documents along with it can be bank statements, pay stubs, W2's, tax transcripts, ect.

Also it isn't the I-864, it is the form I-134. the I-864 is used for the AOS phase once inside the US after the marriage. I-134 form is used for the interview stage for the K-1 visa.

Yes you would need the form I-134 for the interview at Juarez Consulate, but is not necessary you send nothing after your form 129F is approved.
