I just want to clarify how can I form & put AKA the correct name to avoid 221g from USEM. Let me explain the situation here. My wife was annulled to her first marriage so I'm her second current marriage here in the Philippines with IR1 petition process which her interview could come up for next month or so. I was told some NBI offices wont help until USEM letter shows first with 221g. Is this for real?

Her birth single name: N G C

Her 1st marriage ( annulled ) N C D

Her 2nd current marriage N C S

Current the NBI Clearance shows... Family name: C    Middle name: G     First name: N    Husband's surname: S    ( That is what her current name is except the middle name no longer used with G )

Now, Do I need to put FULL annulled name=  N..... C..... D..... or just D.... under AKA??? Really, I don't understand for some reasons. I am feeling so embarrassed..! :help::help:

Every name used with every spelling. Use current legal name and have search done all AKA's. NBI will do the searches but you have ask and list the names to search.

Yes you need to list you previous married name, your current named name and your birth name.  3 FULL names.

Today, I went to NBI with my wife and reissued a new one with AKA. At first, I did not mention what it was for to avoid asking for USEM letter so they accepted the process until just last minute a lady who handled the finger printing asking about AKA with previous married name. She then checked PSA certificate of the annotation, etc... and SAID,,,, Where is the USEM letter? I told her no interview date and was told to put AKA to avoid 221g. Well,,, she said that the letter is required to verify the AKA. Somehow, a guy who assisted us to the end probably convinced her to hand us the new NBI I assumed and FINALLY got new clearance, blue colored multi purpose, no derogatory record, with AKA previous married name. I would not be surprised if 221g comes up for AKA if more than just previous married name. She does not have any nickname anyways.... What I thought was that the NBI is supposed to do their job to search all names to form & input the correct AKA names. WTH?! So far,,, it is full name of the previous marriage started with N. C. D. ( her first name, family name, and previous surname ) for AKA with other information the current status " married ", my last name S.... ( husband's surname), Family name C...., Middle name G..... ( mother's maiden name ) with " no derogatory record " rather than no pending or criminal on record. Thank god.... Let's wait and see after the interview :ph34r:
