Everything was going so well not until my Fiance went for his Medical last week. He was advised that he has to go through the Sputum Test. I was just wondering on how would it affect our petition? The approval notice that the USCIS sent us is only valid for 4 months dated March 14 -July 14. So then the test result will not be out till July or worst case scenario if it comes back positive(which we’re hoping won’t happen :( ) a minimum of 6 month tx. Do we have to re apply again? We’ve been trying to call the embassy to verify the situation, but we can’t get through with any of those numbers they gave. He does not have an interview date yet as we we’re trying to get all the req. first before we set it up. Do we call them and advice them about the situation? Thank You!

Depends on the country.  Usually, you can schedule and go ahead with the interview and then wait for the medical to clear.  I know in the Philippines, the treatment for a positive test last 6 months.  After which there is another test and if everything is clear and the interview went well, then the visa is given.  Try emailing the embassy.  Sometimes that's an easier way to get an answer.

The embassy should automatically renew the approval while you await a clean medical. You can contact them to confirm as well.

Sputum will take ~2 months. If negative, you should hear back afterwards. If positive, you will hear back sooner and need to undergo ~6 months of treatment + a new medical.

Edit: For reference, some embassies (Manila included) may not permit you to actually interview if you have not completed the medical. There are people who have been turned around around and told to complete the medical first.
