I am the beneficiary, and I have my interview in a few weeks. 

My petitioner recently moved, after our petition was approved.

Do I have to bring something to the interview saying he moved, or just tell them?

Does he have to fill something out?

He should just do a regular change of address with USCIS.  The only address they'll use in the immediate future may be yours if they send the passport and visa via a service.  Though you will need to update anything with your future US address for other documents.  Since you file for your green card and social security number later, you should have a chance to update as you go. Also in POE they ask you to confirm your US address.
