Lyn,

You are not alone. I have not received my Packet 4 as of yet and I'm worried that they didn't find my address when they tried to deliver it. Follow my advice... Call St. Lukes and tell them about your concern or go there personally and take the medical and while you're there, tell them to request the Packet 4 for you at USE.

Bring these, for you to have the medical:

NOA2 or print out of your appointment from USE website

Passport

2 x 2 pictures

Fee

I advised this to one of our fellow VJers here and she did the medical without her Packet 4 last Friday. She was told that she can come back Monday for it but at the end of her medical, it was handed to her.

I hope I was of help...

Good luck!

Mae
