Along with the AOS, I have to submit a copy of the form I-94 Arrival/Departure record (Both sides). I believe the form is attached to the passport. Do I have to detach it to make a copy of the back side or should I just send a copy of the Front side only. Anyone know?

Do exactly as it says, copy both sides.

Detach it from your passport, copy it, then re-attach it.
