Do I need to get medical screening the second time before applying for AOS(green card) to complete my I-693 form( one of the requirements)? Or I can only have my vaccination part filled in by an approved civil surgeon and leave other parts of the I-693 form blank? My whole package with all medical results was left at a port of entry upon arrival, thats why I don't have copies of the whole medical on hand, but I DO have vaccination sheet and CD with my X-RAY. We got confused by the information they give in official instructions and that info which approved doctors refer to. Thank you for all help in advance

In the I-693 instructions is your answer:

3 What if I am a K nonimmigrant visa holder and already had a medical examination overseas?
If you were admitted as a:
A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or
B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and
C. You received a medical examination prior to admission, then:
(1) You are not required to have another medical examination as long as you file your Form I-485 within one year
of an overseas medical examination; and
(a) The panel physician did not find a class A medical condition during your overseas examination; or
(b) The panel physician did find a class A medical condition, you received a waiver of inadmissibility, and
you have complied with the terms and conditions of the waiver.
(2) Even if a new medical examination is not required, you must still show proof that you complied with the
vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included
as part of the original overseas medical examination report, you will have to have the Part 9. Vaccination
Record completed by a designated civil surgeon. In this case, you must submit Parts 1., 2., 3., 4., 6., and 9.
of Form I-693.

Also, the topic is mentioned in AOS guide here in VJ http://www.visajourney.com/content/k1k3aos

If your med is less then 1 y old and your vaccination record DS 3025 is completed you just include a copy of the record in your packet, mentioning that in a cover letter wouldn't hurt too.

Thank you very much for the reply, we do have the DS 3025 completed with a stamp on it from the Kiev Medical center in Ukraine. Question is how do we know that its completed properly?

As per instructions (If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the Part 9. Vaccination Record completed by a designated civil surgeon. In this case, you must submit Parts 1., 2., 3., 4., 6., and 9. of Form I-693.")

Just trying to see if we need to go to civil surgeon to fill out then vaccination form (Part 9)

Thank you very much!

What says part 2 of your DS3025? That’s the part the panel physician put a checkbox regarding your vaccination that you either voluntary competed all of them or you are waved for some of them and good to go etc.

Generally if you agreed and took all suggested vacc shots you are good to go. Most Europeans countries citizens are waived from certain vaccinations bcs some of the diseases are not common in our area.
