Question about DS-3025 - page 1 of 2

I was just putting everything together and noticed my DS-3025 vaccination form has only 1 page. I noticed at the bottom of the page saying "page 1 of 2". There's also a note at the middle-bottom saying "Please complete Page 2". 

I'm quite sure I only got one page from the clinic ( I had it in the same folder since I got it and the CD is also with it), unless I somehow gave it to the officer when I entered USA, but I very much doubt that.

 

I'm currently waiting for a friend to get home and help me phone the clinic where I had the medical done at, maybe they can help me and email it. But otherwise has someone been through something similar? Is it possible they didn't hand it to me because it's not important?

 

The page I have looks exactly like the one attached (I googled the form)

I was given a 1 page DS-3025 form as shown above.

I called them last night and the lady there asked me to send an email with my request. It sounded like she didn't know what's up.

I didn't receive an answer to that yet. I'll probably end up sending it without it. This thing made me doubt myself wether or not I lost the page somehow.
