You put NRA (Non Resident Alien) in the place of the SSN which then makes the return not fileable electronically.  If you go to the tax forum this has been asked a 1000 times already

YES...You will have to print form and mail it in. I was told that when they arrive. Then you will amend tax to include the info. I guess. That is what I did.

Not if filing separately.  If you want to file joint then the W-7 would be required

YMMV

No impact either ways because there are legitimate reasons to file either way, a spouse not having a TIN is certainly one

YMMV

No.  You cannot ever get a spouse or dependent deduction for anyone without a TIN

What I did for tax years 2015 and 2016 using Turbo Tax was:

1.  Filled spouse SSN field with 000-00-0000

2.  Printed forms

3.  Used whiteout over fake spouse SSN

4.  Hand wrote "NRA" in spouse SSN field

5.  Mailed forms.

As @payxibka said, do not claim spouse exemption if spouse does not have SSN nor ITIN....
