On April 26, I have oath ceremony, and I also made US passport appointment online as well the same day after ceremony.

I am applying for Passport book and the passport card, my question is should I make 1 check to "US department of State"  or 2 separate checks ? 1 for Passport and 1 for passport card?

 

I am taking my Certificate, my DL, passport photos, and passport form with me.

 

Anything else I am missing?

A lot of times at the oath ceremony they have all these things available right there. I've been to a few oath ceremony here in Las Vegas,NV down at the Federal courthouse and I occasionally volunteer to help out. After the ceremony you can get a photo with the judge, and they have tables set up with people to help you fill out your forms for the passport and get it mailed in. They also have people helping you register to vote, and they even have people from the social security office to get you a new card if you need it. Is a really amazing event. Usually about 200 people here taking the oath, and I always like seeing all the different Flags they people hold representing their country they are from ( They are the small handheld flags). But to answer your question you can use just one check for the total fees. I have done both myself, and renewed multiple times, and have always sent just one check. So maybe you won't need that appointment and you can get everything done after the oath ceremony. And depending where you are going for the passport, if you are going to your local post office, they will get you a money order for the total fees after reviewing the application if you need the payment.

According to my friend who is Immigration officer at my local USCIS office said " we only do maximum 60 people " , and my ceremony is at Federal reserved park, some kind of cave :) About. 3 hours away from where I live.

USCIS office is less than a mile away from my house, but my ceremony is 3 hours away:)

If you use the form filler on state.gov it will tell you who to make the checks out for and for what amount based on the options you picked (DS-11 vs. DS-82, regular or expedited service, card, book, or both, etc.)


I applied via DS-82 (renewed existing passport on Apr 9th) and all fees (book + card + expedited processing + Priority Mail Express return delivery) were included in a single check to the Department of State. Application was accepted and I have all documentation (cancelled existing passport book & card / new passport book / new passport card) in hand as of today.

DS-11 is a little bit different as there is a fee to be paid to the acceptance agent. Again though, form filler should distinguish this and tell you the check amount to write to the acceptance agent and the amount to send to Department of State based on what you picked.

Oh we have the same date for the oath! I wanted to go to the passport office right away too but my local post office didnt have any available spots that day so I will have to wait until Monday.

To answer your question, the official government website has a fee calculator telling you how much and you have to write your check for that specific fee. Please note that the fee for the post office is separate so dont include that. Here is the calculator:
