Our case was last updated April 6, 2018 from Administrative Processing to Refused. Is this the end of the battle? We're so disappointed about this as the whole visa application have been so exhausting, as we all experienced. However, could it be updated as refused because of our expired medical exam? We haven't received an instruction from the embassy to re do our medical examination. Please we really need some input.

REFUSED means you are still the running. They most likely need more info and might be your medical as you suspect. DENIED is what you don't want to see.

Yes, that is a very likely scenario. You will probably be asked to do the medical again.

Take into consideration that the medical has to be valid when you enter the US (after approval of visa). Medicals are valid for 6 months so there are a lot of timings to take into account.

you will need to know why refused to overcome the refusal

it may be temporary and they will request additional doumentation

as said above,  the medical exam must be current and it may be the only reason
