Re-send papers

Hey Guys!
Another silly question.
I sent all the forms with the fee waiver petition, and (obviously) it came back with negative answer.
All my forms came up with a paper in the front page explaining about the negative.
My doubts are:
 
- Do I take this page off when I sent the forms again with the payment checks, or leave it there?
- Do I take of the Fee Waiver form and put the checks instead, or leave the form there and just put the checks in the front page?
- They changed all the order that I put the forms and proof, Do I have to leave it that way, or can I reorganize all the paper to better fit the proof with the forms?

Don't rearrange anything they've done. 

 

For a clearer history I would leave all for history and put the payment on top maybe with a letter saying that you are remitting the payment requested.
