Need Help with I-130 Questions

Hello,

 

I'm filling out I-130 Form for my husband in the DR. We got married in the Dominican Republic, I am now back in the states beginning the visa process. When filling out the I-130 Form and other forms (Cover letter for I-130 packet, future forms, etc.) should I be using my Maiden or Married name?  When it calls for "Family Name" I've put down my Maiden. Also, my husband uses the last names of both his father and mother. I plan to take his father's last name. Should I be putting down both last names on the form when filling out my husband's beneficiary info?
