130 and 130 a - extra information page

Hi Everyone,

 

Me again  Finally got to the last page of the form - Have another question about it.

 

In the extra information page for both 130 and 130a  we have 3 boxes  - Page number. Part Number and Item Number. I am clear on Page number and part number.

 

But for Item number - lets say when I am filling extra physical address  or extra employment- what would be my item number? 

 

4a - 4 h is Physical address no. 1 and 5a and 5b is date from and date to.

 

6a - 6h is Physical address no. 2 and 7a - 7b is date from and to

 

So in the EXTRA INFORMATION sheet does my ITEM NUMBER become  just 4a - 5b (since I am going to fill address no. 3 and the date for that) or does it become 6a - 7b.  I put 4a - 5b and 6a - 7b  since we will be answering the range of questions in that item number

 

I have done it in a couple of ways and am sharing my page - can you help us understand what is the correct way (preferred)?

I wouldn't worry about the item number.  Just start the text field with "address history continued"

We did it the same way and it was no problem. So don't worry!
