It asks if Rose has documentation of receiving vaccinations according to US Law.  She has no documentation but it asks for an explanation.  Do I state she has no documentation but she will get shots at medical?

I can't speak for Rose's country, but in our case (Taiwan), my wife received the required vacs during the medical.

That is what they do in the Philippines too but the question is asking if she has documentation of vaccinations per US law.  She doesn't but she will.  If I answer "no" I have to explain.  I guess I will just do that.  I was wondering if "yes" is the correct answer since the DS-160 is actually presented after medical.

So will have documentation for the vaccines by that time or she won't? If she doesn't just put no and write "Don't have documentation" as the explanation. If she doesn't have them they will just give them to her.

She will have the DS-3025 after the medical .. you say YES.

Do remember the DS-160 is designed for all non-immigrant visas .. most don't complete medicals.
