n-400 Part 8 - Online Schools Enrolled In

How do I fill-in in Part 8 of n-400 form with the online school I enrolled? I'm residing in Virginia, USA and four years ago I enrolled for online courses in the University of the Philippines Open University which is located in the Laguna, Philippines. 

 

a) Should I type this on the school name and indicate that it is an online program: University of the Philippines Open University (Online - Masters in Distance Education)

 

b) Should I type the physical address of the school in the Philippines or my address in VA where I'm studying at home? 

 

Please help. Thank you.
