Applying for Permanent Residency_Medical Records Needed?

My wife and I are filing for her permanent residency.

Some of the documents needed are vaccination records from her doctors in Russia.

Does anyone know what is exactly needed?

Vaccination Supplement. Hopefully, she has her record of shots from being a kid, and can translate it for a Civil Surgeon. Otherswise, she will need a few shots.

Follow the guide in the AOS section. No matter what happens, it will cost a few bucks.

Provided she had her medical check within the last year (which she should've on the K-1/K-3) all she'll need to submit with the AOS packet is an I-693A Vaccination Supplement. In order to get that, she'll need a civil surgeon (approved by USCIS, you can find the list of approved CS's in your area through the guides section) to sign off on it. The one we went to, and I'm pretty sure most others as well, will want to see some kind of proof from a doctor in Russia that she's been vaccinated. To do that, you've gotta show them the DS-3025 (Vaccination Record) that was filled out at her medical check.

I hope she got a copy. Mine didn't, so we had to have her sister scan and send one. If she doesn't have a copy, she'll have to track one down through the medical office, or start all over.

Pretty much though, all you need is that civil surgeon to sign the sheet and seal it in an envelope.

Most important thing: It needs to be sealed in an envelope, signed or stamped across the seal. We got a RFE because the surgeon signed below the seal.

That's strange, as many people are sending just copies of the DS-3025 or even no vaccination information and are getting approved. I sent just a copy of the DS-3025 and the interviewer looked at it and said it was fine.

Like everything else it depends on who looks at your case.

Or, it could be that your wife was up to date on all of her shots. Mine needed to get a few that weren't on the DS-3025 (or, at least weren't completed on it) that were on the I-693A.

The DS-3025 (Department of State, form #3025/Immunization Record) is slightly different from the I-693A (Immigration form #693A/Immunization Record (Supplement)). There are certain shots that are required for AOS that weren't necessarily required for the interview, or that were not completed for the interview. That's where the civil surgeon comes in. (S)He has to certify that all required immunizations have been completed. If they were completed prior to the interview and arrival in the U.S., it shouldn't need to be "certified" that they've been completed, it's already on the DS-3025.

It does clearly state in the directions for the AOS (I-485) a copy of the I-693A is necessary. The necessity of that form, if proof of all immunizations is already established with a DS-3025, seems to be pretty low. If you know she's had all of her immunizations then you probably don't need to go to the civil surgeon. But, if you don't know, then you better go.

(On a side note: Prior to Neonred's post about sending only the DS-3025, I had never heard of doing that. But, it makes sense that if you've already established proof, why send more?)

Yes Slim, over in the AOS section there are several people that sent in just the DS-3025. I had my fiancee get all her vaccinations in Moscow at the medical so we wouldn't have to deal with it later. Looks like it was a good plan. A friend of mine in Indiana sent their AOS off without any vaccination record and were approved thru CSC (not that I would advise it).

If the vaccination record is incomplete then you probably have no choice but to get the shots and have the I-693a filled out.
