I-693a in Sealed envelope

The civil surgeon didn't seal Marlene's 693a in an envelope. I submitted it along with the rest of our AOS package -- just the completed form without an envelope. It concerned me a bit that it wasn't in a sealed envelope, but it didn't cause any delay or RFE for our AOS.

I really don't think it matters if it is in a sealed envelope. A lot of us were approved by just sending in a copy of our DS-3025. Nothing sealed and no special forms from a civil surgeon.

Yes, some people who apply for AOS and don't send in a Vaccination Supplement do get approved. However, if you are going to send one in, or are RFEed for it then it needs to be in a sealed envelope. This is so that they know a Civil Surgeon filled it in. As mentioned before, anyone could fill it in and sign it, so this is a way to ensure it was done properly.

I sent in the Vaccination Worksheet for myself, received no RFE, and got approved.

I got the I-693A (Vaccination Supplement) done for my daughter, they gave us a photocopy and the sealed envelope, cost $35.00. The envelope said "Do not open" on it.

On the main page for Form I-693, the first sentence under "Note on where to file says, "If filing for adjustment of status at a Service Center, you should submit the completed Form I-693, sealed in an envelope from the physician, with Form I-485 package to the Service Center."

Well...it is supposed to be sealed. But you were also supposed to get a copy.

We had a Civil Surgeon here in Austin transfer the records and mail us the sealed envelope + a copy, for only $10.00.

The CV we went to gave us a copy of the supplemental along with the sealed one. We mailed the sealed one in with the application and didn't have any problems.
